다음은 지난 6월 14일 부산대학교 병원에서 개최되었던 부울경 지회 대한세포병리 슬라이드 세미나에 제출되었던 증례입니다.

병력: 79세 남자환자가 복통, 오심 및 구토를 주소로 내원하였다. 복부 전산화단층촬상 소견상 복막의 전이성암종이 의심되었으며, 복수에 대한 세포병리검사와 진단적 복강경 생검을 시행하였습니다.

다음은 대표적인 복수 세포병리슬라이드의 사진입니다.

토의자는 아래와 같이 세포학적 소견을 기술하였습니다.
1) Presentation: High cellularity, single population
2) Tissue Fragments:
- 3-dimensional cell clusters with single dispersed cells
- Variable sized loose clusters
- Berry-like knobby border
3) Cells
- Mostly small sized
- Some large sized cells
- Well defined to fuzzy membrane
4) Nucleus
- Centrally and some eccentric located round to oval shaped with fine chromatin

- Crisp and smooth nuclear membrane - Multi/Bi-nucleation Inconspicuous or single prominent nucleoli

- Atypical mitosis
5) Cytoplasm
- Abundant cytoplasm and vacuolation
- Peripheral cytoplasmic snouting
- Dense biphasic cytoplasm
6) Background
- Inflammatory, neutrophilic and lymphocytic
- Not necrotic

토의자는 다음과 같은 질환을 감별하였습니다.

ㆍMetastatic adenocarcinoma

ㆍMalignant mesothelioma, epithelial type

ㆍReactive mesothelial hyperplasia
